Qualifications

• Must hold a Master’s degree in Social Work.
• LSW licensure is required; LISW preferred.
• Experience working in hospital or community settings recommended.

Responsibilities

• Assess social, emotional, financial, cultural and family needs and communicate these to the care team.
• Assist with linking to community resources and provide guidance for patient concerns.
