Job summary

STAT Home Health is seeking a PRN Licensed Medical Social Worker to serve Escambia/Santa Rosa County, FL. The role emphasizes patient assessments, in-home hospice care, and coordination with families and care teams.

The candidate should have a Master’s in Social Work, an active Florida license, 1–2 years of LMSC experience, and reliable transportation for in-home visits. Hospice or home health background is preferred, with strong communication and empathetic, people-focused service.
