Job summary

STAT Home Health is seeking a PRN Licensed Medical Social Worker to serve Okaloosa/Walton County, FL. The role focuses on patient assessments, ongoing hospice care, and discharge planning, with an emphasis on compassionate, well-documented case management.

Qualifications

Responsibilities
