Job summary

Amedisys is seeking a Home Health Medical Social Worker in Pulaski, Virginia, to join our team on a per diem, Monday–Friday schedule. You will support patients and families with emotional, social, and financial needs while coordinating with the care team in the home setting.

Responsibilities include implementing the plan of care, communicating status changes, and educating families on social work interventions.

Qualifications

• Requires master's or doctoral degree in social work.
• Minimum one year of social work experience in health care.
• Meets state and federal requirements for this position.
• Current CPR certification.

• Provides social work services in accordance with the plan of care.
• Ensures changes in the patient's clinical status are communicated to the healthcare team and care center staff in a timely manner.

• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient's environment and coordinates appropriate interventions with the physician and healthcare team.
• Other duties as assigned.

Pay Range: $29.00/hr to $36.00/hr

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.
