LifeChoice Hospice & Palliative Care is seeking an experienced Hospice Social Worker to join our interdisciplinary team.

Hospice Experience Is Required For This Position.

The Social Worker will support hospice patients and families with emotional, social, financial, and practical needs throughout end-of-life care. This role works closely with nurses, physicians, chaplains, CNAs, families, facilities, and other members of the hospice care team.

Responsibilities
