Abundance of Love Home Healthcare, LLC is seeking a qualified Medical Social Worker to provide psychosocial assessments and support to patients in their homes within the agency's service area.

Responsibilities include collaboration with nurses and physicians, development and modification of care plans, and assistance with resources and referrals to promote patients' independence and well-being.

Qualificações

• Bachelor's or master's degree in Social Work from an accredited program.
• Current, unrestricted Illinois LSW or LCSW license in good standing, as applicable to services provided.
• CPR certification; online-only CPR not accepted.

Responsabilidades

• Conduct psychosocial assessments and assess factors affecting health, daily functioning or coping.
• Identify barriers to maximum benefit from medical care.
• Provide social work services within scope of practice and treatment plan.
• Support patients and families in understanding and following medical recommendations.
• Assist families with personal, social and environmental difficulties affecting health and safety.
• Help patients access family, healthcare and community resources.
• Provide information, referrals and assistance with social-service programs.
• Support adaptation to illness, disability, and changing living circumstances.
• Coordinate with interdisciplinary team and participate in care planning.
• Document and report changes in patient social or emotional status.
