Job summary

Western North Carolina Community Health Services (WNCCHS) seeks a full-time Medical Social Worker/Medical Care Coordinator based at Minnie Jones Health Center, 257 Biltmore Ave, Asheville, NC 28801. This role coordinates assessment, care planning and advocacy to improve health outcomes for underserved patients.

The position requires 2 years of care/case management experience, strong motivational interviewing skills, and knowledge of FQHCs. Excellent benefits and no weekend work are provided.

Qualifications

• 2 years previous experience in care/case management.
• Motivational interviewing skills and knowledge of stages of change interventions.
• Familiarity with the role of FQHCs in the community.

Responsibilities

• Coordinate the assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet health needs.
• Promote patient safety, quality of care and cost-effective outcomes.
• Communicate with patients and available resources to meet comprehensive health needs.

Education

Bachelor's degree in human services, social work or psychology; Master’s degree preferred
