Job summary

Western North Carolina Community Health Services (WNCCHS) seeks a full-time Medical Social Worker/Medical Care Coordinator to guide assessment, planning, facilitation, care coordination and advocacy for individuals and families at Minnie Jones Health Center in Asheville, NC.

You will use motivational interviewing and care-management skills to promote patient safety, quality of care and cost-effective outcomes within our FQHC setting, with benefits including comprehensive health plans and paid

Qualifications

• Bachelor’s degree in human services, social work or psychology; Master’s degree preferred.
• 2+ years experience in care/case management.
• Expert knowledge of medical office operations.
• Familiarity with the role of FQHCs in the community.

Responsibilities

• Coordinate assessment, planning, facilitation, care coordination and advocacy for options and services.
• Promote patient safety, quality of care and cost-effective outcomes.
• Based at Minnie Jones Health Center, Asheville, NC.
