Qualifications

• Master's Degree from a CSWE-accredited program.
• Current CPR certification.
• Licensed Social Worker in the state of residence.
• Valid driver's license, vehicle insurance and access to reliable transportation.
• 1+ year(s) of social work experience in a health care setting.

Responsibilities

• Understand significant personal, emotional, environmental and social factors related to the patient's health status.
• Contribute to development of a comprehensive Plan of Care with the health care team.
• Instruct team on community resources to assist patients as needed.
• Serve as Bereavement Coordinator and plan bereavement services with clear delineation of services.
