The Trinity Health Medical Social Worker (MSW) in home health provides counseling, care coordination, and access to community resources for homebound patients and their caregivers. Working with physicians and care teams, the MSW helps overcome social and environmental barriers to health, safety, and independence.

This position offers a $5,000 sign-on bonus and supports compassionate, patient-centered care in a collaborative, mission-driven environment across Ohio’s Columbus region.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program required.
• Current Social Work licensure as required by state regulations.
• Minimum two years of healthcare social work experience preferred.
• CPR certification required.
• Strong assessment, counseling, communication, and care coordination skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Provide counseling and emotional support to patients and caregivers facing illness, disability, or life transitions.
• Connect patients and families with community resources, financial assistance programs, and support services.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Educate patients and caregivers on coping strategies, available resources, and care options.
• Monitor patient progress and adjust care plans as needed.
• Maintain accurate and timely clinical documentation.
• Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives.
• Support patients with complex social and emotional needs to improve health outcomes and independence.
