Job summary

Trinity Health At Home is seeking a Medical Social Worker (MSW) to provide counseling, care coordination, and community resource support to homebound patients and their caregivers. You will collaborate with physicians and interdisciplinary teams to overcome social and environmental barriers impacting health and quality of life.

This full-time position requires an MSW, current licensure, CPR certification, and strong assessment and communication skills.

Qualifications

• MSW from an accredited program.
• Current Social Work licensure required by state regulations.
• Minimum two years healthcare social work experience preferred.
• CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Provide counseling and emotional support to patients and caregivers facing illness, disability, or transitions.
• Connect patients and families with community resources, financial assistance programs, and support services.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Educate patients and caregivers on coping strategies, resources, and care options.
• Monitor patient progress and adjust care plans as needed.
• Maintain accurate and timely clinical documentation.
• Participate in interdisciplinary team meetings, care conferences, and quality improvement initiatives.
• Support patients with complex social and emotional needs to improve health outcomes and independence.
