Stockton Hematology Oncology Medical Group, Inc. is seeking a behavioral health specialist to support oncology patients and families with psychosocial assessments, counseling, and coping strategies related to cancer.

The role involves connecting patients with community resources, coordinating care with the oncology team, and educating on advance directives and palliative options, all while maintaining patient-centered, culturally sensitive care and thorough EMR documentation.

Responsibilities

  • Conduct psychosocial assessments for oncology patients and families to identify emotional, financial, transportation, housing, caregiver, and support needs.
  • Provide short-term counseling, emotional support, crisis intervention, and coping strategies related to cancer diagnosis and treatment.
  • Assist patients and families with adjustment to illness, treatment side effects, prognosis discussions, and survivorship planning.
  • Connect patients with community resources, financial assistance programs, disability resources, transportation services, support groups, hospice, home health, and other supportive services.
  • Collaborate closely with the interdisciplinary oncology care team to coordinate patient care and address barriers to treatment compliance.
  • Provide education regarding advance directives, goals of care, palliative care services, and available support resources.
  • Facilitate referrals to mental health providers, psychiatric services, substance abuse resources, or additional supportive services when appropriate.
  • Maintain timely and accurate documentation within the electronic medical record (EMR).
  • Participate in multidisciplinary meetings, case reviews, and care coordination discussions.
  • Advocate for patient needs while promoting dignity, cultural sensitivity, and patient-centered care.
  • Support patients and families through difficult conversations, disease progression, and bereavement when needed.
  • Maintain compliance with HIPAA, organizational policies, and applicable regulatory standards
  • Any other job duty asked by your manager or administration
