Job summary

40HRS New York is seeking an experienced Licensed Clinical Social Worker with Oncology experience to join its multidisciplinary clinical treatment team in New York. The role focuses on psychosocial support and recovery throughout cancer care.

The LCSW will conduct assessments, develop and monitor treatment plans, and provide individual and group therapy, while ensuring compliant documentation and coordinated discharge planning with families and care teams.

• Oncology experience required.

Responsibilities

• Provides a comprehensive psychosocial assessment of patients' needs and develops treatment plans.
• Delivers effective behavioral health treatment and education to patients, including individual and group sessions.
• Completes all documentation in a timely fashion per hospital policy.
• Ensures services meet licensure board requirements.
• Develops and revises treatment plans based on patient progress and needs.
• Provides comprehensive case management and connects patients to needed resources.
• Assesses Social Determinants of Health and coordinates appropriate referrals.
• Collaborates with patients, families and care teams to plan safe, effective discharge.
• Reassesses progress regularly and updates plans as clinically indicated.

This position requires Oncology experience, working with or in an Oncology Department.

The Licensed Clinical Social Worker treats patients by utilizing a variety of modalities as a member of the multidisciplinary clinical treatment team. Also provides psychological/social interventions that support patients and families relating to behavioral health, levels of care, hospitalization, disease and illness and wellness and recovery.

Responsibilities:

A day in the life of a Licensed Clinical Social Worker includes:

• Provides a comprehensive assessment of a patient's psychosocial needs, develops treatment plans and treatment plan reviews based on findings and according to department policy.
• Provides effective behavioral health treatment and education to patients, whether including assessment or therapy (individual and/or group), and shares and interprets information to staff, families, and/or other approved agencies.
• Completes all documentation written reports in a timely fashion in accordance with department and hospital policy. as required by the Medical Center or Agency policy.
• Provides services to patients in accordance with Licensure regulations defined by the respective licensure board.
• Utilizes and obtains and interprets information in terms of patient needs, knowledge of growth and development, and an understanding of the evidence- based treatment options available range of treatment required for specific populations that age groups, when developing treatment plans.to develop a treatment plan specific to patient needs..
• Provides comprehensive case management services/assessment as warranted.
• Assess Social Determinants of Health (SDOH) and provide information, education and/or referrals as appropriate to meet identified patient needs.
• Collaborates with the patient (with their consent), family, and other support persons involved in the patient's care to develop a comprehensive discharge plan that best meets the continuing care needs of the patient.
• Reassesses treatment plan on an ongoing basis, evaluates progress, and revises plan when clinically indicated.
