Job summary

Interim HealthCare Inc. is seeking a Medical Social Worker (MSW) for home health services in the United States. You will provide comprehensive psychosocial assessment, care planning, and support to patients and families in alignment with physician plans and goals of care.

The role emphasizes collaboration with an interdisciplinary team, compassionate patient-centered care, and improving satisfaction by addressing social and emotional factors in health transitions across the home setting.

Qualifications

• MSW degree required.

• Minimum 3 years of social work experience in a health care setting.

Responsibilities

• Collaborate with Goals of Care to improve patient/family satisfaction and coordinate hospice transitions when appropriate.
• Assess psychosocial status and deliver care per the physician's plan of care.
• Aid team members in understanding social, ethical, and emotional factors related to health problems.
• Communicate with the interdisciplinary team to coordinate patient care across transitions (home to hospital, hospital to post-acute, back home).

Status: Pay Per Visit

As a Medical Social Worker, you will be called to care when you're needed most. As part of Interim health, you'll support a full range of patient services to bring comfort and dignity to our clients.

As a Medical Social Worker, you will:

• Collaborate with Goals of Care team to develop a robust goal concordant program for Interim clients with hopes of improving patient/family satisfaction, decreasing hospitalizations, and facilitating hospice transitions when appropriate
• Assess psychosocial status, establishing, monitoring, and delivering care as it relates to meeting the needs of the patient/client, as directed by the physician's plan of plan of care
• Aid other team members in understanding the social, ethical, and emotional factors related to health problems
• Effectively communicate with other members of the interdisciplinary healthcare team to promote coordination of patient care concordant with patient goals. When necessary, facilitate communication of patient status and plan of care during transitional experiences such as home to hospital, hospital to post-acute care facility, and back home

To qualify as a Medical Social Worker with us, you will need:

• Education: Master of Social Work (MSW) degree
• Licensure Requirement: MSW in the state in which the position is located
• Minimum 3 years of social work experience in a health care setting
