Job summary

Interim HealthCare of Augusta seeks a Home Health Medical Social Worker to visit patients in Verona, VA and surrounding areas, connecting them with resources, counseling and support to improve quality of life. You will collaborate with an interprofessional home health team to address psychosocial needs and support patient-centered goals.

The role requires a Master’s degree in Social Work and preferred Virginia MSW licensure, with CPR certification and experience in home healthcare.

Qualifications

• Master’s degree in Social Work is required.
• Active MSW license in Virginia preferred.
• Minimum of 2 years of social work experience, ideally in home healthcare.
• CPR certification is required.
• Knowledge of state and federal home health regulations.
• Strong interpersonal, communication, empathetic and resourceful skills.

Responsibilities

• Provide home-based social services to patients in Verona, VA and surrounding areas.
• Collaborate with an interprofessional home health team to support patient goals.
• Monitor psychosocial condition and identify social and emotional needs.
• Conduct patient assessments, document progress and progress toward goals.
• Provide counseling, crisis intervention and advocacy, and connect families with resources.

As a Home Health MSW, here’s a big-picture view of what you’ll do:

• Provide home-based social services to patients dealing with depression, anxiety, poor nutrition or a lackof resources that make managing their injury or disease difficult
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT, OT or SLP, focused on the patient’s plan of care and individual goals
• Monitor a patient’spsychosocial condition and identify social and emotional needs
• Conduct patient assessments, document progress and ensure patient is moving toward goals
• Provide counseling, community resource planning, crisis intervention and advocacy Consult with family and caregivers on patient’s plan of care and how to help them progress

A few must-haves for Home Health Medical Social Workers:

• Master’s degree in Social Work
• Active MSW license in Virginia preferred
• Minimum of 2 years of social work experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
