Essential Functions

• Conduct comprehensive psychosocial assessments to identify barriers related to mental health, social determinants of health (SDOH), housing, food access, transportation, financial strain, and caregiver support.
• Provide brief counseling, crisis intervention, and motivational interviewing to support patient engagement in care plans.
• Collaborate with Rendr360 nurse care managers and providers to coordinate care for high-risk patients.
• Develop individualized care plans addressing psychosocial, behavioral, and community resource needs.
• Facilitate referrals to behavioral health services, community agencies, and government programs (e.g., Medicaid/Medicare benefits, home care, housing support).
• Document all encounters in the electronic medical record according to care management and compliance standards.
• Support transitional care, chronic care management, and disease management programs by addressing psychosocial risks contributing to hospital utilization.
• Participate in interdisciplinary meetings, case discussions, and quality improvement activities.
• Advocate for patients and families to ensure access to appropriate care and services.

Qualifications

• Master’s Degree in Social Work (MSW), required.
• Licensed Master of Social Work (LMSW), preferred.
• 1 year of medical or healthcare social work experience (hospital, primary care, home care, or care management settings), preferred.
• Trilingual proficiency in Cantonese, Mandarin, and English required.
• Experience working with diverse, immigrant, and older adult populations strongly preferred.
• Strong communication, problem solving, and care‑coordination skills.
• Ability to work collaboratively in a multidisciplinary team environment.
