Qualifications

Position: Medical Social Worker

NHC HomeCare Greenwood is looking for a Medical Social Worker to join our team. The Medical Social Worker supervises and provides evaluation of the social and emotional needs of the patient related to their medical condition, and provides counseling, etc. based on this evaluation.

Qualifications - ALL states:

• Master’s Degree from a school of social work accredited by the Council on Social Work Education.
• One year social work experience in a health care setting is required.

PLUS state-specific requirements:

South Carolina specific

• Licensed by the South Carolina Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.

Position Highlights:

• Assess social and emotional factors to minimize impact on the patient’s health and ability to cope with problems of daily living.
• Help patients and caregivers understand, accept, and follow medical recommendations.
• Assist patients and caregivers with personal and environmental difficulties that predispose toward illness or interfere with obtaining maximum benefits from medical care.
• Identify resources, such as caregivers and community agencies, to assist the patient to resume life in the community, including discharge planning.
• Assist providers and home care team members in understanding the significant social and emotional factors related to the health problems.
• Participate in the development of the plan of care.
