Job summary

NHC HomeCare Greenwood is seeking a Medical Social Worker to supervise and evaluate the social and emotional needs of patients related to their medical condition, and to provide counseling based on this evaluation.

The role involves performing comprehensive social assessments, coordinating with the home care team, and helping patients and caregivers navigate care plans and discharge planning to maximize independence and quality of life.

Qualifications

• Master's Degree from a CSWE-accredited school of social work.
• One year of social work experience in a health care setting.
• Licensed by the South Carolina Board of Social Work Examiners at the LMSW or LISW level.

Responsibilities

• Provides and documents direct social services to patients to restore optimal social and health adjustment.
• Assesses social and emotional factors to minimize impact on health and daily living coping.
• Helps patients and caregivers understand, accept, and follow medical recommendations.
• Assists patients and caregivers with personal and environmental difficulties related to illness and care.
• Identifies community resources to aid patient discharge and resumption of life at home.

• Provides and accurately documents direct social services to patients planned to restore the patient to optimum social and health adjustment.
• Assesses social and emotional factors to minimize impact on the patient’s health and ability to cope with problems of daily living.
• Helps patients and caregivers to understand, accept, and follow medical recommendations.
• Assists patients and caregivers with personal and environmental difficulties that predispose toward illness or interfere with obtaining maximum benefits from medical care.
• Identifies resources such as caregivers and community agencies to assist the patient in resuming life in the community, including discharge planning.
• Assists providers and home care team members in understanding significant social and emotional factors related to health problems.
• Participates in the development of the plan of care.

Qualifications

• Master’s Degree from a school of social work accredited by the Council on Social Work Education.
• One year of social work experience in a health care setting.

South Carolina State-specific Requirements

• Licensed by the South Carolina Board of Social Work Examiners at the Master (LMSW) or Independent (LISW) level.
