Job summary

Diamond Care Group is seeking a compassionate Medical Social Worker to support home health patients and their families, addressing psychosocial, emotional, and resource needs. You will work with a dedicated interdisciplinary team to improve patient outcomes.

You will assess psychosocial needs, assist with discharge planning, provide counseling, connect families with community resources and financial assistance, and ensure Medicare/home health documentation compliance.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Florida license (LMSW/LCSW preferred)
• Home health or healthcare experience preferred
• Strong knowledge of community resources
• Excellent communication and organizational skills
