Qualifications

  • LCSW or Oregon Clinical Social Work Associate; licensure within 3 years.

  • Valid driver’s license, insured vehicle, and clean driving record.
  • Excellent communication, flexibility, creativity, and problem-solving with minimal supervision.
  • Adherence to NASW Code of Ethics.

Responsibilities

  • Serve as active member of hospice IDT; collaborate on individualized plans of care.
  • Conduct comprehensive psychosocial assessments for patients and families.

  • Prepare and begin discharge planning upon admission for home hospice and house patients.
  • Provide counseling, crisis intervention, and emotional support to patients, families, and caregivers.
  • Maintain accurate, timely documentation in accordance with policies and regulatory standards.
  • Communicate clearly: verbal, written, telephone, and electronic with daily IDG updates.
  • Maintain knowledge of current community resources and resources navigation.

Job Summary

The Medical Social Worker is an essential member of the hospice interdisciplinary team, providing psychosocial support and services to patients and families in accordance with the plan of care at the Pete Moore Hospice House and in patients’ homes. Care is delivered under a physician’s orders and in alignment with hospice philosophy. This role involves evaluating patients’ psychosocial needs, including care giving planning, implementing therapeutic interventions, and initiating discharge planning upon admission. The Medical Social Worker demonstrates sound judgment, psychological sensitivity, and adherence to all regulatory and accreditation requirements. The position may include supervising students and interns, assisting with new staff orientation, and ensuring compliance with the National Association of Social Workers (NASW) Code of Ethics. Must be available to assist with weekend admit on the home team, and cover days at the Pete Moore Hospice House

Education & License Requirements

Master’s degree from a school of social work accredited by the Council on Social Work Education. Licensed Clinical Social Worker status required or an Oregon Clinical Social Work Associate will be considered with licensure completion required within three years of hire date.

Minimum Qualifications & Experience

Experience with terminally ill patients and their families is highly desirable. Must have current driver’s license, a licensed vehicle for transportation, an insurable driving record, and a means of communication. Requires excellent communication skills, flexibility, creativity, and the ability to problem-solve with little supervision.

Essential Job Functions

  • Organize, prioritize, and delegate at a fast pace.
  • Assist in developing care giving plan or guide and assist in the Medicaid process.
  • Maintain accurate, timely documentation in accordance with organizational policies and regulatory standards.
  • Communicate clearly: verbal, written, telephone, and electronic, with daily updates to IDG.
  • Maintain knowledge base of current community resources.
  • Work within organizational processes and policies.
  • Work independently, utilizing good time management.
  • Negotiate terrain and physical obstacles to reach patients, drive a vehicle safely.
  • Participate in conflict resolution, problem solving, peer communication, and crisis management.
  • Participate in interdisciplinary team meetings and care conferences.
  • Demonstrate sound judgment, maintain confidentiality.
  • Assist patients and caregivers with interface to community and government organizations.
