Fond du Lac Band of Lake Superior Chippewa's Human Services – Medical Services division in Minnesota seeks a full-time Social Work Specialist to assist clients during inpatient medical stays, coordinate DME, and navigate insurance coverage.

You will conduct intake, develop care plans, coordinate discharge planning, and advocate for patients across multi-disciplinary teams, ensuring culturally appropriate care and smooth transitions.

Division/Department: Human Services – Medical ServicesLocation: Center for American Indian Resources & Min No Aya Win Human Service CenterReports To: Director - MedicalProbationary Period: 90 days – Full time, 120 days – Part timeFLSA Status: Exempt (Salaried)Classification: Child-Related CADriver’s License: Level 2- Driver’s License required.Rate of Pay: $29.47 to $33.60Bonus: $5,000 with 2-year commitmentResponsibilities: Assists clients in navigating inpatient medical stays.Provide support for patients to experience culturally appropriate care while in an inpatient settingAssist Clients in obtaining Durable Medical Equipment (DME).Accept, record and ensure completion of referrals from other providers outside the agency.Conduct intake interviews with clients.Develop a plan of care with client/families when appropriate.Offer options within network when appropriate.Identify and secure alternative resources for clients.Provide follow-up services to client/family.Participate in inter-professional team-based primary care casework analysis and planning.Exchange pertinent information with team members as necessary.Provide feedback to referring provider when appropriate.Coordinate placement in rehabilitation programs such as discharge after hospitalization or treatment facilities for substance abuse when necessary.Assist with discharge planning from the hospital using a shared decision-making approach with patients and families to support safe transitions to home, rehabilitation facilities, assisted living, or long-term care as appropriate.Assist patients with obtaining insurance coverage for long term care and helping them understand the policy’s coverage.Assist with coordinating patient care conferences. Work with staff to schedule care conferences with providers, outside agencies and appropriate family members or POA if applicable.
