Job summary

Amedisys, one of the largest home health and hospice providers in the U.S., seeks a Medical Social Worker in Virginia with an active VA LCSW license. This full-time daytime role focuses on helping patients heal at home and offers flexibility, purpose and growth within the Amedisys team.

Compensation ranges from $61,000 to $75,000 annually with a point pay system after orientation and a productivity goal of twenty points per week.

Qualifications

• Masters or doctorate degree in social work from a CSWE-accredited program.
• One year of social work experience in a health care setting.
• Active Virginia Licensed Clinical Social Worker (LCSW).
• Current CPR certification.

Responsibilities

• Assist the patient and family with health related financial, social and emotional concerns and assist with effective coping skills.
• Provide social work services in accordance with the plan of care.
• Ensure that changes in the patient's clinical status are communicated to other health care team members and office staff in a timely manner.
• Educate the patient and family and other team members on social work interventions and treatment plan.
• Perform care planning, transfer and discharge functions.
• Identify risks or threats to the patient's health in the patient's environment and coordinate appropriate interventions with the physician and team.
• Other duties as assigned.

• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.


• Educates the patient and family members and other team members on medical social work interventions and treatment plan.


• Performs care planning, transfer and discharge functions.


• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.


• Other duties as assigned.

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education


• Has one year of social work experience in a health care setting.


• Meets state and federal requirements for this position.


• Possess a clear and active Licensed Clinical Social Worker license in the state of Virginia.


• Current CPR certification.
