Summary:

The Medical Respite Program allows individuals who are experiencing homelessness a safe place to rest and recuperate from a medical condition. While participants focus on improving their health, they can also work on addressing other needs such as Social Determinants of Health , including housing. The Medical Respite Coordinator is responsible for overseeing the clients admitted to the NSO Medical Respite Program and assisting with care coordination activities following discharge from the hospital, as well as the Medical Respite site. This position will help clients navigate the healthcare delivery system, while working to address barriers to permanent housing. Collaboration with other professional disciplines is required to ensure effective and efficient client care delivery and the achievement of desired client outcomes.

Responsibilities

  • Coordination of all medical and social services following the clients discharge from the hospital and the medical respite site
  • Assist with coordination of all onsite telehealth appointments for mental and physical health services
  • Coordinate of all medication deliveries from local hospitals and pharmacies
  • Identify and refer clients to local community resources
  • Provide coordination of food services provided to patients
  • Assist with patients transition to next housing location.
  • Participate in daily interdisciplinary care team meetings.
  • Communicates respectfully and professionally , with all members of the care team, to provide patient-centered care
  • Act as a patient advocate and liaison between the patient/family and community service agencies.
  • Supports organizational initiatives to promote and maintain a strong positive workplace culture.
  • Respects patient confidentiality at all times and treats patients with courtesy and respect.
  • Performs other duties as assigned by supervisor, contractual obligations and/or program description

Candidate Requirements

  • HS Diploma
  • 60 Hours of College credit hours (Preferred)
  • Experience working with underserved population
  • Experience with identifying, accessing and referring to local community resources
  • Proficiency in MS Office (Word, Excel, PowerPoint & Outlook)
  • Ability to learn additional software
  • Proficiency in general office equipment (PC, printer/fax/copier, telephony system)
  • Experience with electronic health records and scheduling systems
