James River Home Health and Hospice is seeking a detail-oriented Medical Records Specialist to join our administrative team in Virginia. You will manage clinical documentation, process physician orders, and ensure compliant record-keeping to support quality patient care.

Qualifications

• 1-2 years of healthcare administrative, medical records, or related experience preferred.
• Home Health or Hospice experience strongly preferred.

Responsabilités

• Track Start of Care (SOC) documentation and follow up with clinical staff to ensure required paperwork is received within established timelines.
• Process, monitor, and track physician orders to ensure compliance with Medicare, Medicaid, and private insurance requirements.
• Follow up with physician offices regarding unsigned physician orders and outstanding documentation.
• Maintain and monitor Face-to-Face encounter documentation requirements.
• Maintain physician database information, including demographic updates, licensure verification, and PECOS verification.
• Process medical record requests and ensure records are released accurately and within required deadlines.
• Maintain strict confidentiality of patient information and medical records in accordance with HIPAA and company policies.
• Create, maintain, update, and purge reports and data as requested by leadership.
• Respond to internal record requests and assist departments with documentation-related inquiries.
• Provide backup phone and reception coverage as needed.
• Assist with the distribution of clinical supplies when necessary.
• Support department goals, process improvement initiatives, and company growth efforts.
• Perform additional duties and special projects as assigned.
