Responsibilities
Under general supervision, provides medical records services including but not limited to preparing and maintaining paper and electronic medical records, filling medical records requests, assisting in office functions where needed, creating new patient packets, copying and scanning documents, and researching, retrieving, inspecting, and entering information in the electronic medical record.
- Responsible for the scanning/importing of all medical records to patient record.
- Ensures department upholds Federal and State laws, rules and regulations.
- Where assigned, sorts, verifies, and places unsigned loose reports such as disability paperwork, FMLA, etc., or added patient visit records in appropriate physicians’ box for review and signature. Accurately documents the patient’s Name and Medical Record Number on all records. Scans into patient chart after signature/completion by physician.
- Responsible for completing, recording, and invoicing of all medical record releases (ROls) including but not limited to Continuity of Care, patient requests, insurance requests, third-party vendor requests, chart reviews, legal requests/subpoenas, disability/FMLA.
- Prepare fee slips and labels for patient appointments, allowing for a smooth and expeditious patient visit and flow of billing processing.
- Manages all incoming medical records communication (email, faxes, and phone) to include inserting records into patients EHR, routing referral information to central scheduling, completing ROIs, sending faxes to physicians and techs as appropriate.
- Always maintains accuracy of 98% for all imported medical records. Report errors to updating pool for correction.
- Handling of file conversions to ensure syncing of documents to multiple medical record systems.
- Mailing new patient packets and recall letters to patients, providing information beneficial to positive patient experience.
- Must adhere to all legal requirements regarding the safe handling of such information including HIPAA regulations, Ohio state law, OSUP policies, etc.
- Travel may be required to accommodate staffing levels at other clinical facilities.
- Other duties or special projects as assigned.
Qualifications
High School diploma or equivalent.
Preferences
- Experience working in a Medical Office or Customer Service.
- Data entry experience with basic understanding of insurance and billing procedures.

Pay Range
USD $15.05 - USD $22.63 /Hr.
