Job summary

Northwell Health is seeking a Medical Practice LPN in the Primary Care department in Wilton, CT. The position is 40 hours per week with 8-hour shifts from 7:45am to 5:15pm, Monday through Friday.

The LPN provides direct patient care under supervision, supports exams, administers medications and vaccines, and assists with specimen collection and testing. Travel between practices may be required.

Qualifications

• Active LPN license in state; BLS certification within 30 days.
• HS graduate or equivalent; graduate of an accredited LPN program.

Responsibilities

• Prepare materials, supplies, and patient to assist clinician with exams.
• Obtain patient history and medical information.
• Provide direct patient care as directed by RN or clinician.
• Collect and process specimens and perform point-of-care testing.
• Administer medications and vaccinations; instruct on preventive care.
• Contact patients for follow-up and relay test results or instructions.
• Maintain compliance responsibilities and uphold the organization's values.
• Demonstrate regular, dependable attendance.
• Travel between practices as needed.

Medical Practice LPN position in the Primary Care department in Wilton, CT.

40 hours per week, Monday-Friday, 8-hour shifts between the hours of 7:45am-5:15pm.

Summary

To assist the clinician in providing medical care to the patients of the Medical Practice.

Responsibilities

1. Prepares materials, supplies, equipment, and the patient to assist clinician with the patient's medical exam.
2. Obtains the patient's history and medical information.
3. Provides direct patient care as directed by a Registered Nurse or clinician.
4. Collects and processes specimens as directed by the clinician and performs point of care testing.
5. Administers medications, and vaccinations as directed by the clinician. Instructs the patient and family on how to perform clinician recommended preventive care, takes prescribed medication, prepares for tests, monitors treatment progress and notes symptom changes.
6. Contacts the patient to provide requested follow-up, advises them of test results or relays clinicians instructions.
7. Fulfills and adheres to all compliance responsibilities related to the position.
8. Maintains and Models the Organization's values.
9. Demonstrates regular, dependable, and predictable attendance.
10. Performs other duties as required.

Other Information

Ability to travel between practices as needed.

Required

Active LPN License based on state; BLS certification required within 30 days.

Education Required

• HS Graduate or Equivalent.
• Graduate of an accredited LPN Program.
