Job summary

Northwell Health in Connecticut seeks a dedicated LPN to assist clinicians in a Medical Practice, located at 100 Redding Road, Redding CT. The role is 40 hours weekly, Monday–Friday, with 7:45am–5:30pm shifts.

The LPN will travel between practices as needed, perform direct patient care, collect specimens, administer medications, and communicate test results. BLS within 30 days and active LPN license required.

• Collects and processes specimens as directed by the clinician and performs point of care testing.
• Administers medications and vaccinations as directed by the clinician; instructs patient and family on preventive care and preparation for tests.
• Contacts patients to provide follow-up, relay test results or clinician instructions.
• Fulfills compliance responsibilities and maintains organizational values; demonstrates dependable attendance.

Medical Practice LPN position in Primary Care Office at 100 Redding Road, Redding CT.

40 hours per week, Monday-Friday, 8-hour shifts between the hours of 7:45am-5:30pm

Summary

To assist the clinician in providing medical care to the patients of the Medical Practice.

Responsibilities

1. Prepares materials, supplies, equipment, and the patient to assist clinician with the patient's medical exam.
2. Obtains the patient's history and medical information.
3. Provides direct patient care as directed by a Registered Nurse or clinician.
4. Collects and processes specimens as directed by the clinician and performs point of care testing.
5. Administers medications, and vaccinations as directed by the clinician. Instructs the patient and family on how to perform clinician recommended preventive care, takes prescribed medication, prepares for tests, monitors treatment progress and notes symptom changes.
6. Contacts the patient to provide requested follow-up, advises them of test results or relays clinicians instructions.
7. Fulfills and adheres to all compliance responsibilities related to the position.
8. Maintains and Models the Organizations values.
9. Demonstrates regular, dependable, and predictable attendance.
10. Performs other duties as required.

Education Skills Experience

Ability to travel between practices as needed. Required: Active LPN License based on state; BLS certification required within 30 days.
