Job summary

Lincare Holdings Inc. in Houston is seeking a field-based patient care coordinator responsible for evaluating patient adherence to physician orders and supporting home enteral therapy.

You will educate patients, perform assessments, coordinate with clinics, and ensure insurance coverage while traveling to patients' homes to deliver equipment and provide care. The role requires strong communication and organization to ensure high-quality patient outcomes.

Qualifications

  • Read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Calculate figures and amounts such as commissions, proportions, percentages, area, circumference, and volume.
  • Solve practical problems and deal with a variety of concrete variables in situations with limited standardization.
  • Interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Should have knowledge of word processing, spreadsheets, and email communications.

Responsibilities

  • Provides patient education as part of patient care activities according to patient needs and policy.
  • Performs complete and professional patient assessments per policies and procedures.
  • Maintains current knowledge of enteral home care techniques and concepts.
  • Represents the company through professional appearance, patient care activities, and written reports.
  • Gathers information from referral sources to ensure insurance coverage of each enteral patient; follows up on paperwork to ensure payment for services rendered.
  • Maintains patient records in complete and organized manner per accreditation standards.
  • Drives to admit, deliver, and pick up enteral formula, feeding pumps, IV poles, and related supplies to patients' homes or business locations.
  • Provide patient care, treatment, and service in a continuous and coordinated manner to meet patient needs.
  • Promote safe, effective patient and organizational environments, as well as feeding equipment use.
  • Participate in ongoing education including in-services and trainings to maintain competence.

Job description

This employee performs patient followup, generally responsible for the evaluation of patient adherence to physician orders.

Job Responsibilities:

  • Provides patient education as an intricate part of the patient care activity, according to the patient need and policy
  • Performs complete and professional patient assessments in accordance with policies and procedures
  • Maintains current knowledge of enteral home care techniques and relevant enteral therapy concepts
  • Represents the company through professional personal appearance, patient care activities, and content of written reports
  • Gathers necessary information from referral source to ensure insurance coverage of each enteral patient
    • Follows up on necessary paperwork to ensure payment for services rendered
  • Maintains patient records in complete and organized manner in compliance with accreditation standards
  • Drives to admit, deliver, and pick up such items as enteral formula, feeding pump(s), IV poles, and/or related feeding supplies to patients' home or place of business
  • Provide patient care, treatment, and service in a continuous and coordinate manner in settings appropriate to meet patient needs
  • Promote safe, effective patient and organizational environments, as well as feeding equipment use
  • Participate in on-going education that includes in-services, training, and other activities to maintain and improve individual competence
Qualifications.

Education and Experience:

  • One year experience in home health care, hospital, clinic, or nursing home strongly preferred
  • Experience/Education in Dietetics, Nutrition, Home Health, or Nursing strongly preferred
  • One year of experience as an LPN, DTR, or CNA preferred
  • Must have excellent human relations skills to interface with doctors, other referral sources, and patients

Physical Demands:

The employee must occasionally lift and/or move up to 50 pounds.
