Job summary

Lincare Holdings in Marlton, NJ seeks a patient-focused clinician to evaluate adherence to physician orders and support enteral home care. You will conduct assessments, educate patients, and coordinate care with referral sources.

Responsibilities include maintaining records, ensuring insurance coverage, transporting enteral supplies, and promoting safe care in home settings. Strong communication and health-care background preferred.

Qualifications

  • One year of experience in home health care, hospital, clinic, or nursing home strongly preferred.
  • Experience/Education in Dietetics, Nutrition, Home Health, or Nursing strongly preferred.
  • One year of experience as an LPN, DTR, or CNA preferred.
  • Must have excellent human relations skills to interface with doctors, other referral sources, and patients.

Responsibilities

  • Provides patient education as part of the patient care activity, according to the patient need and policy.
  • Performs complete and professional patient assessments in accordance with policies and procedures.
  • Maintains current knowledge of enteral home care techniques and relevant enteral therapy concepts.
  • Represents the company through professional personal appearance, patient care activities, and content of written reports.
  • Gathers necessary information from referral source to ensure insurance coverage of each enteral patient; follows up on paperwork to ensure payment for services rendered.
  • Maintains patient records in a complete and organized manner in compliance with accreditation standards.
  • Drives to admit, deliver, and pick up enteral formula, feeding pumps, IV poles, and related supplies to patients' home or business.
  • Provide patient care, treatment, and service in a continuous and coordinated manner in settings to meet patient needs.
  • Promote safe, effective patient and organizational environments, as well as feeding equipment use.
  • Participate in on-going education that includes in-services, training, and other activities to maintain and improve competence.
  • Read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Calculate figures and amounts such as commissions, proportions, percentages, area, circumference, and volume.
  • Solve practical problems and deal with a variety of concrete variables in situations with limited standardization.
  • Interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Job description
