Medical Director - Post-Acute Care Management - Care Transitions - Remote

We are seeking a Medical Director for Post-Acute Care Management focusing on Care Transitions. This role is remote, allowing work from anywhere within the US during daytime hours across all continental time zones.

Primary Responsibilities
  1. Provide daily utilization oversight and communicate with network physicians and hospitals.
  2. Conduct utilization management reviews, authorizations, and denial reviews.
  3. Engage in peer-to-peer clinical case reviews and provider telephonic discussions.
  4. Ensure effective communication with providers to facilitate care transition services.
  5. Respond to clinical inquiries and serve as a medical contact point.
  6. Represent Care Transitions externally, establishing relationships with thought leaders.
  7. Collaborate with client services and analytics teams to improve services.
  8. Participate in medical advisory boards and provide leadership in cost management and care coordination.
  9. Contribute to strategic initiatives and monitor progress.
  10. Provide scheduled weekend and evening coverage as needed.
Minimum Qualifications
  • Board certification as MD, DO, or MBBS with an active, unrestricted license.
  • At least 3 years of post-residency patient care experience, preferably in inpatient or post-acute settings.
Preferred Qualifications
  • Multiple state licensure and willingness to obtain additional licenses.
  • Knowledge of population-based medicine and CMS criteria for post-acute care.
  • Strong communication, organizational, and interpersonal skills.
  • Experience with electronic health records and technology applications.
  • High ethical standards and adaptability in a fast-paced environment.

The role offers a competitive salary range of $286,104 to $397,743 annually for residents of California, Colorado, Connecticut, Hawaii, Nevada, New Jersey, New York, Rhode Island, Washington, or Washington, D.C. The salary includes benefits, incentives, and stock options, with specifics based on experience and location.
