Job summary

Legacy Care is seeking a Medical Director/Physician to lead clinical practice and deliver high-quality care in post-acute settings around Richmond, VA. You will guide care plans, mentor teams, and ensure regulatory compliance while promoting continuity of care and positive patient outcomes.

Ideal candidates hold an MD/DO with board certification, an active Virginia license, and DEA registration. Experience in post-acute or geriatrics is preferred; leadership experience is a plus.

Qualifications

• MD or DO degree from an accredited medical school.
• Board certification in Internal Medicine or Family Medicine.
• Active, unrestricted Virginia medical license.
• Current DEA registration.
• Experience in post-acute care, long-term care, geriatrics or related clinical setting preferred.
• Previous clinical leadership or medical director experience preferred.
• Strong clinical judgement, communication and organizational skills.
• Ability to work autonomously and collaborate with an interdisciplinary team.

Responsibilities

• Provide medical care and clinical oversight in post-acute care settings.
• Assess patients, develop and review care plans, and monitor clinical progress.
• Provide clinical leadership and guidance to medical colleagues and care teams.
• Collaborate with nursing staff, advanced practice providers and other team members.
• Support high standards of patient-centered care and continuous clinical improvement.
• Ensure care and documentation meet professional, regulatory and organizational requirements.
• Communicate with patients, families, facility staff and care teams about treatment plans.
• Use EMR systems to document care accurately and promptly.
• Support care coordination and transitions between settings as appropriate.
