Job summary

Eventus WholeHealth is seeking an Attending Physician to lead population health efforts in skilled nursing and assisted living facilities under the Outcomes-Centered Healthcare model.

You will collaborate with Advanced Practice Clinicians to deliver high-quality, team-based care, with potential Medical Directorship oversight and sign-on bonus/relocation assistance. Board-certified or eligible physicians with post-acute experience are preferred.

Qualifications

• MD/DO required; board cert in Internal/Family Medicine or Geriatrics preferred.
• CMD certification a plus; required within 2 years if serving in Medical Directorship.
• Active and unrestricted medical license; DEA registration as required.
• Minimum 2–3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred.
• Experience collaborating with interdisciplinary teams and facility leadership.
• Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow.

Responsibilities

• Provide leadership in client facilities and collaborate with APCs to improve patient care.
• Ensure high-quality, team-based care across assigned facilities.
• Potentially serve in Medical Directorship for clinical leadership and oversight.
