Medical Director - Physician

The Personalized Health Partners medical practice is currently recruiting a physician to serve as Medical Director covering Summers Healthcare Center located in Hinton, WV.

As Medical Director, you will be part of an exciting new model of care combining good old-fashioned bedside manner with innovative technologies to reduce hospital readmissions and improve patient outcomes.

Job Duties and Responsibilities:

• Assume administrative authority, responsibility, and accountability for implementing medical services, policies, and procedures according to regulations and current post-acute and long-term care practices.
• Coordinate medical care, including emergency procedures, liaise with attending physicians, and monitor care quality.
• Provide physician services within skilled nursing facilities and/or clinics.

Qualifications:

• M.D. or D.O. required
• Gerontology certification preferred
• MD or DO license in West Virginia
• ABMS Board Certified in Internal Medicine, Family Practice, Geriatrics, or Hospice/Palliative
• 5+ years of experience with geriatric or long-term care populations
• Experience in working with and educating Nurse Practitioners

Preferred Qualifications:

• Healthcare management/leadership experience
• Knowledge of CMS, Medicare Advantage, SNP regulations
• Experience with HCC and risk-adjusted reimbursement
• Familiarity with inpatient/SNF utilization metrics and bed management
• Knowledge of state-specific collaborative practice agreements and midlevel licensing
• Proficiency with EMR systems
