Job description

Location: A city in Indiana with a rich industrial history, particularly in manufacturing and engineering, but has diversified its economy over the years. This city is known for its parks and vibrant downtown area with shops, restaurants, and entertainment venues. It is adjacent to South Bend, 90 miles to Chicago, and 140 miles to Indianapolis.

Facility: Provider designed to offer the entire continuum of care and services to frail seniors, helping them maintain their independence in their home and community for as long as possible.

Opportunity:

  • Supervised by the Executive Director.
  • Provides general medical direction and supervision of participant care.
  • Responsible for clinical outcomes, participant care delivery, and oversight of the QAPI program.
  • Develops and maintains medical policies, procedures, standards, and protocols.
  • Provides leadership and guidance to Primary Care Providers (PCPs) and Advanced Practice Providers (APPs).
  • Requires a driver’s license, proof of insurance, and transportation.
  • Proficient in Microsoft Office Suite and basic email, calendaring, and software tools.
  • Excellent organizational, customer service, flexibility, and problem-solving skills.
  • Willing to travel periodically within the region.
  • Conducts history and physical assessments on new participants and periodic reassessments; develops medical treatment plans accordingly.
  • Evaluates applicants and participates in intake and assessment decisions.
  • Discusses medical conditions and prognosis with participants and caregivers to aid in care planning.
  • Works collaboratively across divisions, participating in the nursing treatment program and interdisciplinary teams.
  • Establishes and maintains plans to achieve operational goals.
  • Participates in team and family conferences, educating participants and families about health maintenance and chronic conditions.
  • Supervises healthcare and treatment programs as developed by the interdisciplinary team.
  • Performs physical examinations, orders and evaluates labs and diagnostics, and records results systematically.
  • Possesses effective physical assessment and chronic disease management skills for older adults.
  • Willing to work in an innovative healthcare setting and manage changing priorities.
  • Qualifies for the federal school loan forgiveness program.
