Job summary

Vitae Health is seeking an experienced physician to serve as Medical Director across a portfolio of skilled nursing and long-term care facilities. You will provide clinical oversight, mentorship, and collaboration to ensure high-quality, patient-centered care.

The Medical Director rotates on-site weekly or bi-weekly with practice support from Nurse Practitioners, delivering leadership for a team-based care model and guiding complex cases to reduce avoidable hospitalizations.

Qualifications

• MD or DO from an accredited program
• Active medical license in practicing state(s)
• Board certification in Internal Medicine, Family Medicine or Geriatrics
• DEA registration and prescriptive authority

Responsibilities

• Serve as attending and supervising physician across portfolio, rotating on-site weekly or bi-weekly
• Provide physician leadership and clinical supervision to Nurse Practitioners
• Collaborate with NPs on individualized care plans and escalations
• Co-sign and oversee NP-delivered care between physician visits
• Provide on-call coverage 7:00 PM – 7:00 AM daily
• Lead medical direction and governance for facilities, supporting QAPI and policy development
• Participate in care plan meetings and quality improvement activities
