Job summary

Vitae Health is seeking an experienced physician to serve as Medical Director across a portfolio of skilled nursing and long-term care facilities. You will lead the Physician NP team, provide clinical oversight, and ensure coordinated, patient-centered care.

The Medical Director works on-site weekly (or bi-weekly) per building, guiding clinical staff and partnering with facility leadership to reduce hospitalizations and improve outcomes.

Qualifications

• MD or DO from an accredited program and active license in practicing state.
• DEA registration and prescriptive authority.
• Board certification in internal medicine, family medicine, or geriatrics.
• Experience leading clinicians in SNF/long-term care settings.

Responsibilities

• Serve as Medical Director for a portfolio of skilled nursing facilities.
• Provide clinical oversight and mentorship to Nurse Practitioners.
• Collaborate with facility leadership on patient care plans and escalations.
• Co-sign and oversee NP-delivered care between physician visits.
• Provide on-call coverage 7:00 PM–7:00 AM daily.
• Lead quality improvement and governance efforts with interdisciplinary teams.
