Qualifications

• MD or DO degree from an accredited medical school.
• 5-7 years of clinical experience in home-based care or complex disease management.
• Leadership experience and proficiency with EMR/EHR systems.

Responsibilities

• Provide clinical leadership and mentoring to Intention providers.
• Ensure adherence to clinical standards and organizational policies.
• Conduct health assessments during home visits.
• Develop individualized care plans and prescribe medications as appropriate.
