The position supports mentorship of nurse practitioners and collaboration with interdisciplinary teams. The ideal candidate has an MD or DO with Ohio license, board certification in IM/FM/Geriatrics, and experience in geriatrics or post-acute care.

Qualifications

• MD or DO with active, unrestricted Ohio medical license.
• Board certification in Internal Medicine, Family Medicine, Geriatrics, or related specialty preferred.
• Experience in skilled nursing, long-term care, post-acute care, or geriatrics preferred.
• Experience collaborating with nurse practitioners and interdisciplinary teams.
• Prior Medical Director experience in nursing homes or post-acute care preferred.

Responsibilities

• Provide medical direction and oversight for resident care, establish clinical standards and best practices.
• Mentor and supervise nurse practitioners, conduct regular case reviews with the interdisciplinary team.
• Ensure compliance with federal, state, and local long-term care regulations, participate in surveys and audits.
• Lead QAPI initiatives to reduce avoidable hospitalizations and improve clinical outcomes.
• Serve as clinical liaison among administration, nursing staff, providers, residents, and families.

Position Description

The Medical Director provides clinical leadership and medical oversight for a skilled nursing and long-term care facility in Fremont, OH. You will set clinical standards, support the management of complex geriatric and chronic conditions, and work closely with our nurse practitioners as their mentor and clinical guide. The role combines onsite rounding with administrative responsibility. This position is available on a full-time or part-time basis, and you build your own hours around what works for you. The salary listed above reflects the full-time arrangement.
