Job summary

Redding Rancheria seeks a Medical Data and Referrals Specialist to coordinate referrals from Tribal Health Systems to external providers. The role focuses on communicating with patients, gathering records, and ensuring follow-up through the EHR and SQL-based tracking.

The position follows a Monday–Friday 8:00 AM–5:00 PM schedule. Entry-level wage is noted; compensation adjusts with experience and qualifications.

Responsibilities

• Receive referrals/requests from providers through the EHR and schedule outside care as indicated.
• Provide information to patients regarding referral provider name, address and phone number to schedule appointments; acquire pertinent records as needed and forward to the specialist or facility.
• Track all referrals sent out to ensure completion of care and receipt of related documentation using SQL reports.
• Maintain all required data in the EHR to comply with clinic, state and federal mandates.
• Request medical records/other information from outside healthcare facilities.
• Respond to authorized requests for information by outside healthcare facilities.
• Receive outside records and scan to the appropriate EHR patient file.
• Scan in-house forms and documents to the appropriate EHR patient file.
• Track stored historical paper charts and shred as policy requires.
• Perform other related duties as needed.

The Redding Rancheria is seeking a Medical Data and Referrals Specialist. The preferred schedule for this position is Monday - Friday 8:00 AM - 5:00 PM.

The listed wage reflects the entry-level rate for this position. Compensation may be adjusted based on experience, education, and qualifications.

JOB SUMMARY:

Coordinate referrals from Redding Rancheria Tribal Health Systems to other health care providers for patient care that is not performed at the Health Center. Referrals are made to providers such as physicians, diagnostic facilities, radiology, laboratories, and hospitals.

ESSENTIAL JOB DUTIES:

• Receive referrals/requests from providers through the EHR and schedule outside care as indicated.

Acquire pertinent records, labs, x-rays, diagnostictestsandhealthsummariesrelatedtothepatient'scurrentconditionfromthemedical records; forward information to the specialist or facility involved.
