Job summary

Vital Heart & Vein in Houston, TX is seeking a meticulous Medical Biller/Coder to join our Revenue Cycle Management team. This role handles CPT and ICD-10 coding, charge entry, and clean claim submission across cardiology and vascular services.

The ideal candidate has a strong coding foundation, knowledge of payer-specific billing requirements, and a commitment to accurate reimbursement and compliance. Experience with eClinicalWorks (eCW) is strongly preferred.

Qualifications

• Minimum 2 years of experience in medical coding/billing for a physician practice, cardiology experience preferred
• Strong knowledge of insurance carrier billing rules for office visits, diagnostic testing, surgical procedures, and interventional services
• Experience with cardiology and vascular Billing is strongly preferred — including echocardiography, stress testing, nuclear imaging, cardiac catheterization, and EP procedures
• Proficiency with practice management and EHR systems — eClinicalWorks (eCW) experience strongly preferred

Responsibilities

• Review and abstract clinical documentation to assign CPT/ICD-10-CM codes for all cardiology and vascular services
• Ensure diagnosis codes capture severity and specificity and correct place of service and provider information on claims
• Cross-reference codes against LCD/NCD guidelines before submission
• Initiate physician queries when documentation is incomplete or unclear
• Stay current with CPT and ICD-10 updates and implement changes timely
• Participate in coding audits to maintain high accuracy
• Enter charges accurately in eClinicalWorks and monitor claim status
• Resolve clearinghouse rejections promptly and ensure pre-authentication and referrals are present

Vital Heart & Vein, established in 1998, is the largest private cardiology practice in the Houston area, dedicated to providing high-quality, compassionate cardiovascular care. The practice includes 40 board-certified physicians specializing in cardiovascular disease, interventional cardiology, vascular and endovascular surgery, advanced heart failure, and podiatry. Vital Heart & Vein offers advanced diagnostic testing and comprehensive therapies for coronary, electrophysiology, vascular, and vein diseases. The practice serves patients throughout the greater Houston area, including Humble, Pearland, West Houston, Beaumont, Northwest Houston, and the Texas Medical Center, using state-of-the-art technology to deliver excellent patient outcomes.

We are seeking a highly skilled and detail-oriented Medical Biller/Coder to join our Revenue Cycle Management team. This dual-function role is responsible for accurate CPT and ICD-10 code assignment, charge entry, clean claim submission, and billing compliance across all cardiology and vascular service lines. The ideal candidate has a strong clinical coding foundation, a thorough understanding of payer-specific billing requirements, and a commitment to maximizing appropriate reimbursement while maintaining the highest standards of compliance.

KEY RESPONSIBILITIES

• Review and abstract clinical documentation to assign accurate CPT, ICD-10-CM, and modifier codes for all cardiology and vascular services, including office visits, diagnostic procedures, interventional cardiology, EP studies, and vascular procedures
• Ensure diagnosis codes reflect the highest level of specificity and accurately capture severity of illness
• Ensure correct place of service, rendering provider NPI, and billing provider information on every claim
• Cross-reference all procedure codes against applicable LCD and NCD guidelines before claim submission
• Initiate physician queries when documentation is incomplete, ambiguous, or does not support the level of service billed
• Stay current with annual CPT and ICD-10 updates and implement changes before effective dates
• Participate in internal coding audits and incorporate feedback to maintain a 95%+ accuracy rate
• Enter charges accurately in eClinicalWorks.
• Monitor and resolve clearinghouse rejections same day
• Verify pre-authorization and referral numbers are present and accurate on all claims before submission

REQUIRED QUALIFICATIONS
