Mainline Health Systems, Inc. is seeking a Clinical Case Manager to join the care team under the Clinic Manager. This role supports a Patient Centered Medical Home model and collaborates with all health services to deliver comprehensive patient care. You will participate in continuous quality improvement and maintain professional communication daily. Typical hours are 8 a.m.
to 5:30 p.m. Monday–Thursday and 8 a.m. to 12:00 p.m. on Friday; schedule adjustments may be required.

The Clinical Case Manager is part of the care team and under the supervision of the Clinic Manager. The Clinical Case Manager primary purpose will be to work in conjunction with all health services within the health center in order to aid in the development of a comprehensive services delivery system aimed at meeting the needs of the patients. The Clinical Case Manager is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of Care.

The salary for this position is negotiated between the individual and the Clinic Manager using established salary scale. Bilingual employees receive an additional $0.50 per hour.
