Our trusted client in Brooklyn, NY, is seeking a Medical Case Manager to support individuals with mental illness in maintaining housing and achieving independence. This temp-to-perm role includes teaching skills, connecting clients to services, and leading wellness group meetings.

Compensation: $24/hour

Location: Brooklyn, NY

Schedule: Monday to Friday, 9 AM to 5 PM

Responsibilities as the Medical Case Manager:

• Client Support: Engage clients with mental illness, assist in community living, and develop housing support plans.
• Group Facilitation: Conduct group meetings focused on wellness management and recovery.
• Service Planning: Complete service plan reviews and regularly assess living conditions for residents.
• Case Management: Collaborate with residents to set and achieve rehabilitation and housing support goals.

Qualifications for the Medical Case Manager:

• Education: Bachelor’s Degree in human services or a related field required.
• Experience: 2+ years working with individuals with mental illness.
• Skills: Computer literacy, and strong writing capabilities for treatment plans.
