Job summary

Qualifications

• Less than 1 year of applicable healthcare experience.
• BLS certification from American Heart Association or Red Cross.

Responsibilities

• Support Physicians, Advance Practice Providers, Registered Nurses and Community Health Workers with important operational and clinical tasks.
• Discuss with patient, family and extended decision makers the benefits of participation with Saint Luke's At Home programs.
• Assist Referral Specialist with coordinating all patient scheduling via EPIC platform for providers and multi-disciplinary care team and scheduling urgent visits for providers from on-call and during business hours.
• Cover other Referral Specialist tasks as needed.
• Provide an excellent patient experience by managing the flow of patients, providing excellent customer service, establishing rapport with patients and maintaining a professional demeanor.
• Conduct pre-visit plans prior to home visits
• Vitals
• Phlebotomy as needed
• Administer injections and coordinate vaccines
• Assist providers with procedures
• Utilize the EMR system and scan/download documents
• Complete various administrative tasks as needed such as prior authorizations, RX refills, etc.
• Other duties as assigned
• Comfort conducting independent home visits; geriatric or primary care experience preferred, or experience in long-term care/skilled nursing facilities

Incumbent in position must have excellent customer service skills and professional demeanor at all times. May be responsible for covering front office job duties as needed.
