Job summary

Carease Health - NJ is seeking a dedicated Medical Assistant or CNA to support our preventive care program, including remote monitoring, CCM, and CoCM. You will take vital signs, assist patients, and collaborate with our clinical team to ensure individualized care.

You will document in electronic health records, educate patients and families, maintain accurate records, and thrive in a fast-paced setting. Relocation before starting is required.

Qualifications

2+ years of experience as a Medical Assistant or Certified Nursing Assistant (CNA) in a healthcare setting.
Certified Medical Assistant (CMA) or Certified Nursing Assistant (CNA) certification.
Reliable and efficient in taking vital signs.
Strong organizational skills and ability to work in a high-volume setting.
Ability to relocate before starting work.

Responsibilities

Take vital signs, perform basic medical procedures, and assist with patient care
Collaborate with our clinical team to develop and implement individualized care plans
Utilize technology and electronic health records to document patient information and track progress
Provide education and support to patients, families, and healthcare providers
Maintain accurate and up-to-date patient records
Work efficiently in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
