Job summary

Start Corporation in New Orleans seeks a Medical Assistant to serve as a patient navigator, conducting telehealth visits from home or in the community, scheduling appointments, and connecting patients to care.

This role requires an accredited Medical Assistant program with certification, a HS diploma, 18+, and a valid driver's license, with willingness to travel outside Houma and Louisiana to meet patients.

Qualifications

Responsibilities

  • Provides outreach to homeless individuals in streets, shelters, and designated sites.
  • Coordinates care, advocacy, housing search, mental health education, daily living skills, and supportive counseling.
  • Coordinates referrals to housing, mental health, substance abuse, medical and social service providers as needed.
  • Maintains contact with service providers to encourage referrals and coordination of services.
  • Maintains electronic records, reports, and statistics for program functioning and evaluation.

Job Description

The Medical Assistant will work as a patient navigator to conduct telehealth visits for patients in their home or meet them in the community. They must be disciplined and function independently in order to schedule and connect patients to telehealth appointments. Must also have an unrestricted valid license and able to travel outside of Houma and Louisiana. They must be comfortable traveling, going into patient's homes, and meeting patients in untraditional clinic settings.

Minimum Requirements
  • High School Diploma or GED
  • Completion of an accredited Medical Assistant Program with certification
  • Be at least 18 years of age
  • Possess a valid Drivers License
Core Competencies
  • Provides primary and behavioral health care to clients of Start’s Health Care Center according to the scope of certification.
  • Administer and monitor medications to clients of Start’s Health Care Center at the clinic and in the community.
  • Communicates respectfully and effectively and establishes professional relationships through active listening and demonstration of empathy and positive regard.
  • Promotes recovery, independence, and personal choice for participants.
  • Assists clients in identifying strengths, needs, and realistic goals to develop a treatment plan.
  • Accurately identifies risk and responds appropriately, and provides consultation to peers regarding high-risk situations and appropriate interventions.
  • Demonstrates advanced knowledge of primary and behavioral health symptoms, diagnoses, interventions, and impact.
  • Maintains professional boundaries and operates within agency / ethical guidelines.
  • Knowledge of agency and community resources and referral processes.
Job Duties / Skills Required
  • Provides outreach to individuals in the streets, shelters, and anywhere designated that homeless individuals may congregate. Whenever they may congregate.
  • Potentially works a non-regular schedule to better meet the needs of the community.
  • Coordinates the care for individuals, providing advocacy, housing search, mental health education, daily living skill training, and supportive counseling
  • Coordinates referrals and linkages to housing, mental health, substance abuse; medical and various social service providers as needed for the individual.
  • Provides testing at no cost to individuals in the community along with education in Harm Reduction.
  • Maintains contact with service providers to encourage referrals and ensure effective coordination of services.
  • Participates in weekly supervision to review cases and issues related to level of service.
  • Attend outreach and other community meetings to coordinate with other outreach workers and community service providers.
  • Collaborate and coordinate services with appropriate community agencies.
  • Develop and maintain excellent working relationships with community partners and work together to identify and improve processes that enhance service.
  • Maintain electronic records, reports, and statistics necessary for program functioning and evaluation.
  • Accept responsibility for professional competency as appropriate.
  • Is familiar with and liaises with hospitals and community agencies.
  • Advocate on behalf of clients and/or support them to advocate for themselves.
