Job summary

Mosaic Gardens at Chandler is seeking a Med Tech to support residents with daily living activities, manage medications, and document care under supervision.

You will take vital signs, assist with bathing and dressing, report changes to the Clinical Manager, and coordinate with outside health professionals as directed. Excellent English and caregiver certification are required, with prior long-term care preferred; evenings, weekends, and holidays may be required.

Qualifications

• Caregiver certification required.
• High school diploma or equivalent required.
• Previous long-term care experience preferred.
• Strong organizational skills and attention to detail.
• Ability to work independently, follow procedures and prioritize tasks in a fast-paced environment.
• Excellent written and verbal communication skills in English.
• Empathy, patience and commitment to resident dignity.
• Flexibility to work evenings, weekends and holidays as needed.

Responsibilities

• Medication Management & Documentation.
• Prepare, administer, and document medications in accordance with Mosaic policies, physician orders, and state regulations.
• Maintain accurate MAR entries and update physician orders when needed.
• Order, receive, and securely store medications per protocols.
• Maintain a clean, organized medication room.
• Resident Care & Support.
• Assist residents with activities of daily living and monitor health changes.
• Take and record vital signs and communicate updates to the Clinical Manager or Executive Director.
• Provide first aid and wound care and coordinate care with outside health professionals.

• Maintain accurate and timely entries in the Medication Assistance Record (MAR).
• Verify, clarify, and update physician orders as needed.
• Order, receive, and securely store medications in compliance with community protocols.
• Maintain a clean, organized, and secure medication room.
• Resident Care & Support
• Assist residents with activities of daily living, including bathing, grooming, dressing, toileting, and ambulation.
• Take and record vital signs, monitor health changes, and communicate updates promptly to the Clinical Manager or Executive Director.
• Provide first aid and wound care when necessary and coordinate care with outside healthcare professionals as directed.
