Job summary

Qualifications

Responsibilities

• Assess patient clinical and psychosocial needs and plan care according to standards.
• Develop, implement and evaluate care plans with patients and families.
• Collaborate with interdisciplinary team to coordinate care and facilitate transitions.
• Educate patients and families and document education and discharge needs.
• Complete clinical and quality documentation per RHS standards and regulations.
• Delegate and prioritize tasks to ensure safe, high-quality patient care.
