Job summary

Hillcrest Hospital South in Tulsa, OK is seeking a day shift, full-time Decentralized Med/Surg Clinical Pharmacist. The role collaborates with nurses and providers to deliver optimal pharmacological care, including discharge rounds, pharmacokinetic review, and medication information services.

Requirements include a PharmD or B.S. in Pharmacy with ACPE accreditation, hospital experience, and an active license. A PGY-1 residency or 2+ years of experience with BLS/ALS/ACLS are also considered.

Qualifications

B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
Clinical pharmacy experience in a hospital setting.
Completion of an accredited PGY-1 Pharmacy residency or fellowship.
2+ years clinical pharmacy experience with no residency or fellowship.
Active Pharmacist license in good standing with State Board of Pharmacy.
BLS & ALS/ACLS certification (BLS within 90 days; ALS/ACLS for Code Blue).

Responsibilities

Serve as a decentralized pharmacist in med/surg areas, collaborating with nurses and providers.
Attend and participate in discharge rounds and provide clinical reviews.
Offer pharmacokinetic consultation and drug information services.
Actively engage in medication-related interventions to optimize outcomes.
Support compliance with applicable state requirements and safety protocols.
