Job summary

Hillcrest Hospital South in Tulsa, OK is seeking a day shift, full-time Decentralized Med/Surg Clinical Pharmacist to join our team. The role collaborates with nurses and providers in med/surg units and participates in discharge rounds to optimize pharmacotherapy.

Candidates should have a PharmD or BS plus ACPE accreditation, hospital pharmacy experience, and be eligible for licensure in Oklahoma. A PGY-1 residency is preferred; alternative is 2+ years with no residency.

Qualifications

• B.S. or PharmD from ACPE-accredited School of Pharmacy.
• Clinical pharmacy experience in a hospital setting.
• Completion of an accredited PGY-1 Pharmacy residency or fellowship.
• Active Pharmacist license in good standing with the State Board of Pharmacy.
• BLS & ALS/ACLS certification (BLS required within 90 days of hire; ALS/ACLS required for attending Code Blue).

Responsibilities

• Serve as a decentralized pharmacist in med/surg with nurses and providers.
• Attend discharge rounds and provide clinical review and pharmacokinetic consultation.
• Engage in medication-related interventions to optimize pharmacological outcomes for patients.

Responsibilities:

This position will serve as a decentralized pharmacist in our med/surg areas working collaboratively with nurses and providers in this area. In addition to traditional pharmacist tasks this role will also attend and participate in discharge rounds, provide clinical review, pharmacokinetic consultation, drug information and actively engage in medication related interventions to ensure optimal pharmacological outcomes for patients.

Qualifications:

Job Requirements

• B.S. or Pharm D. from an ACPE-accredited School of Pharmacy.
• Clinical pharmacy experience in a hospital setting.
• Completion of an accredited PGY-1 Pharmacy residency or fellowship.

OR

• 2+year’s clinical pharmacy experience with no history of residency or fellowship.
• Active Pharmacist license in good standing with the appropriate State Board of Pharmacy.
• Other applicable state requirements - i.e. sterile compounding, etc.
• BLS &ALS/ACLS certification (BLS required within 90 days of hire;ALS/ACLS required for attending Code Blue).

Preferred Job Requirements

• Doctor of Pharmacy from an ACPE-accredited college of pharmacy.
• 1 year of inpatient hospital pharmacy experience
