Job Overview

The Medication Care Manager provides high-quality care by administering medications and treatments safely and organized, complying with all relevant laws and policies, and supporting the Mission to champion the quality of life for all seniors.

Responsibilities & Qualifications

Essential Duties

Supporting our Mission, Principles of Service, and Core Values, the role includes but is not limited to:

Medication Administration

• Administer medications and treatments safely and organized.
• Document medication administration accurately in the electronic health record.
• Follow the Six Rights of Medication Pass.
• Ensure timely medication passing using the mobile med cart.
• Maintain confidentiality and proper documentation for all resident medications.
• Report concerns, medication updates, and discrepancies promptly.
• Assist in restocking and checking medications.
• Assess residents for "as needed" medications and document appropriately.
• Maintain cleanliness and organization of medication areas.
• Follow refill processes and notify supervisors of prescriptions needed.
• Support residents' independence and dignity, practicing good care standards.

Resident Care Communications

• Notify the RCD of any concerns from residents or families.
• Participate in daily meetings and develop Individualized Service Plans.

Risk Management & Safety

• Ensure compliance with health and safety regulations.
• Practice safety procedures, report incidents, and maintain hazard-free environments.
• Follow infection control protocols and proper disposal of hazardous materials.

Training & Team Contribution

• Be a team player, contribute to engagement programs, and participate in meetings and training.
• Maintain required certifications and training as per regulations.

Experience & Qualifications
